The concept of coordinated care stems from the health care field, where the Electronic Medical Record and helps medical professionals efficiently assess patients&#8217; needs in order to provide them with effective, customized care. Now, Âé¶¹´«Ã½ÔÚÏß is adapting this model to create a &#8220;success network&#8221; for each student. Faculty and staff in the success network will collaborate in the platform to support the student.<\/p>\n<span class=\"prpl-button\"><a href=\"https:\/\/montclair.campus.eab.com\/\">Log In to Navigate<\/a><\/span>\n<h2>Catalyst for A Conversation<\/h2>\n<div class=\"prpl-drawer\"><div class=\"prpl-drawer-header\">Key Platform Features<\/div><div class=\"prpl-drawer-content\">\n<p>Perform These Key Actions to Identify, Communicate With, and Support Students<\/p>\n<ul>\n<li><strong>Reference the Student Profile<\/strong> \u2013 After clicking on a student\u2019s name through the search results, your Staff Home, or the Quick Search, note their Academic progress and any areas of concern with the various tabs on a student\u2019s profile.<\/li>\n<li><strong>Add Notes or Advising Summary Reports<\/strong> \u2013 Record your interactions and follow-ups from student meetings by adding an Advising Summary Report (record associated with an appointment) or a Note (general record not associated with a specific meeting).\n<ul>\n<li>Both are accomplished through the \u2018Actions\u2019 menu on your Staff home or search results, or from a student\u2019s profile.<\/li>\n<li>Reminder!
